Different Ears is the AH-side name for what the Fandom infobox calls Mismatching Ears — the CCTV-only anomaly where the patient's ears or horns render differently between the live admission window and the security camera. Per Fandom this is camera-exclusive detection: the live window shows one ear configuration; the CCTV shows another. The mismatch can be in shape, count, or species-appropriate detail (horns vs. ears, for example).
Unlike most CCTV-class anomalies (where the live window is deliberately 'normal' and only the camera surfaces the cue), Different Ears actually requires the live-vs-camera comparison rather than camera-only observation. You need both views to confirm the mismatch — looking at CCTV alone doesn't tell you anything was different, because you'd just see whatever the CCTV is rendering. The window-CCTV comparison is the workflow.
How to Spot It

Three things the ears can do differently
The anomaly isn't just "ears look weird in the photo". The mismatch can take three documented forms — different shape (cat-ear vs rounded), different count (one ear vs two), or different placement (low-set vs high-set). Any of the three is sufficient grounds to reject. Memorise the patient's window-view ears for the half-second you're queuing the Photo Camera, then compare directly.
Photo-class anomaly fundamentals
Different Ears belongs to the six-anomaly photo class — patients who look normal at the live window and reveal their tell only when photographed. The workflow is the same across all six: take the photo for every queued patient, glance at the output, reject on a confirmed mismatch. If you've never had a clean photo come back, you're probably forgetting to take photos at all.
What this is not
This is not the same as a Different Eyes mismatch (separate anomaly, pupil-and-eyelid focused), nor an Incorrect Photo (entirely different animal in the photo). Different Ears specifically locks the mismatch to the ear region. The visual difference between the three photo-mismatch anomalies is fine; the workflow is identical, so as a player you can treat them as one detection step ("photo every patient → look for any anatomy that disagrees with the window") and only worry about the specific subtype when documenting it after the run.
