How a Shift Actually Works
A Shift is one full night at the veterinary clinic. Your team loads into the lobby, buys supplies, picks classes, then patient intake begins. The shift ends when the night timer runs out and every admitted patient has been routed through treatment or discharged. There's no XP or levels — the only progression is the shift number itself.
Per the animalhospital.wiki shifts guide, the first five shifts function as an extended tutorial. After Shift 5 you're in endless survival mode with scaling difficulty.
The 1-3, 4-6, 7-9, 10+ Difficulty Bands
Community consensus across multiple guides groups shifts into four bands:
- Shifts 1-3 — Learning curve. Anomaly tells are generous, environmental enemies rare. The window-vs-CCTV-vs-photo workflow is teachable here without time pressure.
- Shifts 4-6 — Mid-game pressure. Ambulance Event enters the rotation (Shift 4). Paperwork checks (Missing Appointment) start mattering. Skinwalker conversions from missed anomalies become a real run-ender.
- Shifts 7-9 — Veteran nights. The Stalker shows up. Environmental hazards stack. Co-op coordination starts mattering more than individual skill.
- Shift 10+ — Endurance test. Demands role-splits in co-op and weapon readiness. Solo runs at this depth are documented but uncommon.

Why Shift 4 Is the Real Difficulty Jump
Shift 4 is the consensus jump point. Three mechanics activate simultaneously:
- Ambulance Event — six or more patients arriving near-simultaneously. The triage decisions matter for the first time.
- Paperwork-class anomalies become enforceable — Missing Appointment and Mimic Patient (which mimics Ratthew, who started showing up in Shift 2-3) are now active.
- Skinwalker spawn frequency climbs, partly because the paperwork class adds new anomaly types that can convert.
If you're stuck in the 1-3 loop, the bottleneck is usually inventory + class progression rather than skill. Save coins, unlock Surgeon or Security, then push 4.
Shift 7 and the Stalker Era
By Shift 7 the Stalker enemy is in regular rotation. Stalker damages sanity on direct eye contact, which means rounding corners with the camera up costs you. Two adaptations:
- Keep the camera angled toward the floor when transitioning between rooms.
- Run with at least one teammate covering the opposite direction so you're not solo-checking corners.
This is also the band where the Surgeon / Secret Agent class advantage becomes structural rather than just convenient.
What Persists Between Shifts vs What Resets
Persists:
- Animal Coin balance
- Class unlocks (permanent)
- Shop Upgrade purchases (permanent)
- Shift number
Resets each shift:
- Inventory consumables (unless your class starts you with a fixed item like Paramedic's Large Speed Cola)
- Sanity meter
- Active patients on the floor
- Anomaly queue
The persistence model is why the inter-shift Shop window matters — it's the only moment to commit Animal Coins to permanent upgrades.
How to Read the Per-Shift End Screen
End-shift rewards scale with: patients successfully treated, anomalies rejected before entry, team sanity remaining, and event-bonus objectives. The four metrics weight roughly equally, so a "perfect intake but messy treatment floor" shift earns about the same as the reverse.
FAQ
How many shifts are there? No hard cap. The first five are tutorial-tier; everything beyond is procedurally extended endless mode.
Does the difficulty actually cap at Shift 10? No — difficulty keeps scaling. Shift 10 is just the community marker for "endurance phase". Documented runs go higher; most players don't.
Can I skip ahead by buying upgrades? No — shifts must be played sequentially. Animal Coins accelerate your loadout, not your shift number.
Do shop upgrades persist if I lose a shift? Yes. Animal Coins, class unlocks, and shop upgrades are all permanent across deaths.
Where are emergencies on the timeline? Emergencies start at Shift 3. Before that, the loop is intake + treatment only.
