Overview

Shift 3 is the real start of the pressure curve. The tutorial-tier scripting from Shifts 1-2 ends, and three major systems unlock simultaneously: the Supplies Shop (run by Shopkeeper Nurse, see /wiki/npcs/shopkeeper-nurse) for between-shift upgrades, the Emergency Wing (Rooms 6-8 in the left corridor — see /wiki/rooms/room-6-xray, /wiki/rooms/room-7-heart-monitor, /wiki/rooms/room-8-surgery), and the Taser at the Reception Office display case for free hostile-encounter coverage.

The biggest single change is the timed-emergency event rotation. Per /wiki/events/patient-on-fire, /wiki/events/patient-fainted, /wiki/events/death-ritual, /wiki/events/monster-eating-patient, /wiki/events/monster-emerging, and /wiki/events/critical-patient — all six emergency events start firing randomly from Shift 3 onwards. This shifts the cognitive load from 'process patients calmly' to 'manage multiple cascading timers.'

Dr. Harlow's between-shift dialogue (see /wiki/npcs/dr-harlow) is what unlocks the Emergency Wing + Taser progression. Skip his dialogue and the unlocks may not register. Censored Eyes anomaly debuts as a new photo-class threat (see /wiki/anomalies/censored-eyes from Stage E.1).

How to Survive This Shift

Pre-Shift 3 prep: spend any accumulated Animal Coins from Shifts 1-2 at the Supplies Shop. Per the Stage E.2.5 analysis, the highest-value early upgrades are +1 Carry Capacity ($100), Faster Computers ($110), and 50% Faster DNA Analysis ($110). Grab the free Taser from the Reception Office display case before any patient queues. The 5-shot starter X-Taser is Security class's specific bonus, but other classes get a generic Taser variant from the same display. Also grab a Fire Extinguisher from right of the Supplies Shop — it covers Hider + Ghost + fire + Skinwalker counters.

For timed emergency events: each has its own timer (Patient on Fire 60s, Patient Fainted 60s, Death Ritual 40-60s, Critical Patient 100-120s, Surgery Monster 45s in Room 8, Monster Eating Patient 30s with Bed Monster). Memorise the timer-per-event so you can prioritise correctly when multiple fire simultaneously. The 30-second Monster Eating Patient is the shortest budget — that's why pre-stocking Maple Syrup matters once Bed Monster encounters are common.

For Censored Eyes (the new anomaly): the Photo Camera reveals the black bar over the patient's eyes (see /wiki/anomalies/censored-eyes from Stage E.1). Run the Photo Camera intake check on every patient — the 10-sanity cost is non-negotiable now that 4 anomalies are in the active pool.

Clearing Shift 3: successfully discharge at least one patient + survive the random emergency rotation without hitting the 3-strike threshold. Successful completion unlocks Shift 4, where the Ambulance Event debuts.
