Shift 4 is the marquee mid-game difficulty spike. The Ambulance Event debuts here — six patients arrive simultaneously per /wiki/events/ambulance-event, with some already on fire or in critical condition. The hospital transitions from 'manageable shift loop' to 'multi-patient surge cascade,' and players who haven't built Supplies Shop upgrades during Shift 3 will struggle on Shift 4.

The pre-event warning is News Bunny's check-in TV broadcast (see /wiki/npcs/news-bunny). When the news-reporter character appears on the lobby TV, the Ambulance is incoming. Per Stage D's News Bunny entry, this is your prep window — pre-stage Fire Extinguisher (see /wiki/items/fire-extinguisher), Speed Cola (see /wiki/items/large-speed-cola), and Coffee/Chocolate sanity reserves. Position near the Lobby entrance for fastest triage.

How to Survive This Shift

Pre-Shift 4 preparation begins on Shift 3. Spend your Animal Coins on Supplies Shop upgrades — +1 Carry Capacity ($100), Faster Computers ($110), Visitors Check In Faster ($20) all directly improve Ambulance Event throughput.
