Successful completion (without hitting the 3-strike threshold across the entire run) triggers Ron from Accounting's thank-you dialogue, the hospital cleanup ending sequence, and the Professional Caretaker achievement badge unlock. Mechanically, Shift 5 is everything Shifts 1-4 taught you applied simultaneously. All previously unlocked anomalies and enemies are in the active pool.
