Dr. Harlow

Updated:

Supervising physician — runs the briefing dialogue, unlocks features at Shift 2-3, possibly an anomaly himself.

Overview

Dr. Harlow is the hospital's supervising physician and the closest thing the game has to a tutorial voice. He oversees the intern roster, delivers the shift intros, and grades the player's performance at the end of each shift — the post-shift evaluation screen comes from him. He cuts back in between shifts whenever a new mechanic is being introduced: room unlocks, fresh enemy classes, new event types all get a Dr. Harlow dialogue beat.

Mechanically he's tutorial wallpaper. Lore-wise he's significantly more interesting. The Fandom page documents multiple pieces of evidence suggesting Dr. Harlow may himself be an anomaly — he appears as an anomalous figure in a lobby window, some promotional thumbnails depict him with threatening attributes that potentially connect him to the Mass of Eyes incident, and his stated goals align with the Animal Corporation's corporate profits rather than patient welfare. The harsh treatment of interns (which the game leans into with humour) might have a darker explanation: Ron from Accounting's dialogue implies workers die on the job at a rate the hospital actively conceals from new hires.

Appearance & Design

Light orange deer-like creature in blue medical scrubs under a white consultation coat. Black stethoscope around the neck, head mirror with a brown band. Two large antlers in a muted orange tone, slit eyes with white irises, blue surgical mask covering the mouth.

The most distinctive accessory is a black suitcase marked 'TOP SECRET' in red letters, carried on the right hand. The suitcase is visually similar to the one Ron from Accounting carries, which is one of the lore links the wiki uses to suggest the two characters know each other's true roles in the corporation.

Notable Traits

Limited interactions, focused entirely on gameplay mechanics. Very little known about his personality outside of the tutorial-voice role. The deliberately thin characterization is itself a tell — the Fandom page notes his goals align with corporate profits, which suggests his warm-mentor presentation is performative rather than genuine.

Story Role & Lore Significance

Dr. Harlow is the gating NPC for the early-game progression unlocks. After Shift 1, he provides general advice on how the diagnose-treat loop works. After Shift 2, he unlocks new facilities — specifically the Supplies Shop (run by Shopkeeper Nurse) and the Emergency Rooms. After Shift 3, he enables the Taser in the office display case, which is the player's first reliable hostile-encounter weapon before unlocking a combat class.

He stops appearing after Shift 7. The Fandom page does not explicitly state why, but the timing is notable — Shift 7 is widely considered the late-game transition point where the storyline shifts from tutorial-mode to anomaly-heavy content. The 'TOP SECRET' briefcase he carries is the same model that Ron from Accounting carries, which is the wiki's primary visual cue for the broader Animal Corporation conspiracy thread. Both NPCs appear to know things about the hospital that the player isn't told directly.

Fan theory and visual evidence both support Dr. Harlow being an anomaly himself. The lobby-window appearance (anomalous figure visible through a window in the lobby) and the promotional-art depictions with threatening attributes are the main pieces of in-game evidence. The connection to Mass of Eyes (suggested by some thumbnails) would make him the in-fiction source of one of the game's most punishing encounters — but this is fan theory, not confirmed in-game lore. Cross-reference: Ron from Accounting's dialogue corroborates the Animal Corporation conspiracy without naming Dr. Harlow as the anomaly directly.

Trivia & Hidden Details

Dr. Harlow is the appearance basis for both the Doctor and Surgeon playable classes. Doctor uses his base scrubs-and-stethoscope look without the bouffant cap; Surgeon uses essentially the same model with a surgeon cap added. The visual overlap is deliberate — the Fandom page notes Surgeon's model 'seems to be Dr. Harlow with a surgeon cap but they aren't the same person.'

The 'TOP SECRET' briefcase is one of two such suitcases in the game. The other belongs to Ron from Accounting and uses an identical model with the same red lettering. The two-suitcase visual pairing is the primary in-game evidence the wiki uses to suggest Dr. Harlow and Ron from Accounting are both privy to Animal Corporation secrets that the player doesn't fully unlock during normal play.

Frequently Asked Questions

When does Dr. Harlow first appear?
Shift 1. He delivers the opening tutorial dialogue and continues to appear between shifts through Shift 7, after which his appearances stop entirely.
Is Dr. Harlow an anomaly?
Strong fan theory backed by in-game evidence: he appears as an anomalous figure in a lobby window, and some promotional thumbnails depict him with threatening attributes potentially linking him to the Mass of Eyes incident. Not confirmed in dialogue, but the visual evidence is substantial.
What does Dr. Harlow unlock for the player?
After Shift 2: Supplies Shop and Emergency Rooms. After Shift 3: the Taser in the office display case. These are the major early-game mechanical unlocks gated behind his dialogue.
Why does Dr. Harlow carry a 'TOP SECRET' briefcase?
The briefcase is the primary visual link to the Animal Corporation conspiracy thread. Ron from Accounting carries an identical briefcase. The pairing suggests both characters know things about the hospital that aren't disclosed to the player in normal dialogue.
Why do Doctor and Surgeon classes look like Dr. Harlow?
Deliberate appearance-basis link. Doctor uses Dr. Harlow's base scrubs-and-stethoscope look; Surgeon adds a bouffant cap on top of essentially the same model. The Fandom page explicitly notes the visual overlap.
What happens to Dr. Harlow after Shift 7?
He stops appearing entirely. The Fandom page doesn't state a reason, but Shift 7 is the late-game transition point where the storyline shifts from tutorial to anomaly-heavy content. His disappearance is one of the unresolved lore threads.
Is Dr. Harlow connected to Mass of Eyes?
Suggested by some promotional thumbnails that depict him with attributes linking to the Mass of Eyes encounter. Not confirmed in-game, but it's the most-cited fan theory connection.
Does Dr. Harlow have any connection to Barney's storyline?
Indirectly — both characters exist within the Animal Corporation's anomaly framework. Dr. Harlow doesn't appear in the Barney scenes specifically, but the broader corporation conspiracy ties their existence together.
